Fortunately I haven't really had an "OOPS" moment yet. I respect my life too much and respect the danger too much that rock climbing has if you aren't careful.
Whenever I climb I'm safety cautious almost to a fault so my climbing has been thus far uneventful.
Yeah, that would have been a bummer! My first outdoor trip, several years ago, I went with some friends and of course they were leading and cleaning while I just did top rope in the middle.
I don't remember what one of them did, but it was something like that. They were talking about how he didn't set up properly at first and almost unclipped from the anchors.

I had the munter hitched to the gate side and it rolled up and unscrewed the lock and came out ( all in one motion) and thank God I had a tight grip on the rope, and just went down hand over hand, when I got to the bottom I was sick at my stomach. And no I didn't have a back-up, ( stupid move ). It was a short route and I wasn't thinkin, it was early in my climbing before I had alot of experience. That experience was an awakening!!!!
Now, I use back-ups. Anytime you rappell using a conventional method, ( clove,munter,etc...) allways attach them so they work on the back side of the biner.
it was a rock climbing gym training during my first few days as a beginner...... my hands were all sweaty all the time.. i was such a lady being scared and all.... due to the sweat, i'm always having a hard time to take a good grip of the rocks.... i tremble all the time!! good thing i was able to overcome the fear! hahaha

I think I have a new one. I was repelling with my son (7) the other day and I had a good redundant, equalized system. Well while teaching other people how to descend I did not notice my dog was chewing on one side(of three) of our anchor. While my son and I were over the lip my friend noticed that one of the anchors were chewed almost completly through. We were not going to fall but it was still pretty sloppy on my end.

Once, very early in my climbin days, I rapped off a route using a munter hitch and it came out of the biner............
You have to remember to attach it to the locking biner, on the spine side of the biner, not the gate! Duuuuuuuuuh!
